Not just city firms but hospitality and fashion retail also have mandatory heels policies. Being on your feet all day and not having the choice of what you wear is pretty crap. Fine to specify “smart” but that doesn’t necessarily have mean heels.

Only when you’re in a ‘client facing’ situation is traditional business attire required and even that has been relaxed in many companies if they think it appropriate. I used to keep a tie in my desk for when I got an unexpected visitor.

This is not a widespread problem.
