Productivity is simply a relationship between outputs and inputs. So if you halve the number of doctors in a hospital, but only an etxra third of patients die then it becomes more productive.

Of course that is not to say that we should ignore public sector efficiency, but measuring anything more sophisticated will inevitably involve subjective judgements about what to measure, and then probably distort the planning process as managers chase those indicators, just as now they chase targets.
